Waiver 1: Intro Course, accreditations, guests
PLEASE READ: This waiver is for anybody who is:
- Taking an Intro Course or Accreditation at Allez Up
- An unaccredited individual being belayed by one of our members.
Individuals under 18 years of age must have the waiver signed by a parent.

Waiver 2: Children's groups/birthday party
PLEASE READ: This waiver must be filled in by the parents of any underage customer who is being belayed by our staff. A waiver must be signed for each climber. For climbers under the age of 18, a parent or legal guardian must sign. For your convenience a waiver can be downloaded by clicking on the link below:
